Windy Point Chesapeakes Puppy Application Thank you for your interest in our puppies. In order that we may effectively appraise the suitability of placing a Windy Point Chesapeake Bay Retriever puppy with your family, we require you to answer the following questions as concisely and completely as possible. Please remember, there is no right or wrong answer as each person’s situation is unique. If you are interested in one of our puppies, please send this form back to us along with a $300.00 non-refundable deposit to 955 Harding St. Oregon, WI 53575, payable to Patti Maye (deposit will be returned if a puppy is unavailable). Do you still own a dog(s)? If so, how many and what breed(s)? Are you committed to caring for this dog for its lifetime? Do you live in a: house (own or rent), apartment? Urban or rural? Where will your CBR live? Inside, outside, other (please explain further)?? Is a member of the family home throughout the day? If not, what arrangements have been made for the puppy’s care? For what purpose are you buying this breed? Pet, Show, Obedience, Hunting, Field Trials, Hunt Tests, Breeding, Other? Will you obedience train this dog? If so, who will do the training of this puppy?
